If you plan to pay cash or use health insurance

For insurance pay, please include the following:
1. Provider's name
2. Member ID
3. A photocopy of the front and
back of the insurance card
4. Name, date of birth, and address
of the primary on the insurance.
Upon receiving your email, I will send a reply confirming receipt of the information and the next steps.
What to Expect From Sessions
After I have your basic information, I will:
1. Create a profile for you.
2. I will then send you the intake forms to review and fill out. These forms cover my office policies, privacy (HIPAA) rules, and health assessments.
3. Once you complete the paperwork and we check your insurance, we can schedule our first appointment.
