MANDATORY NOTICE
In accordance with 22 TAC §884.31, Texas Health & Safety Code §181.105, and House Bill 4224
1. How to File a Complaint with the Texas Behavioral Health Executive Council (BHEC)
The Texas Behavioral Health Executive Council (BHEC) investigates and prosecutes professional misconduct committed by Licensed Professional Counselors (LPCs), Marriage and Family Therapists, Psychologists, and Social Workers. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint. Filing a complaint will not affect your access to services.

2. How to Request a Copy of Your Mental Health Records
You have the legal right to inspect and receive a copy of your mental health records maintained by this practice.

How to Request: All requests for records must be submitted in writing. You may send your written request via email to cmg@cmg-therapy.com or submit it directly through your secure client portal message system.

Processing & Timelines: Requests will be processed, verified, and fulfilled in full accordance with Texas Health and Safety Code Chapter 611 and HIPAA privacy regulations.
3. How to File a Consumer Complaint with the Texas Attorney General
Pursuant to Section 181.105 of the Texas Health and Safety Code, you have the right to file a consumer complaint regarding health privacy violations or unfair consumer practices with the Office of the Texas Attorney General.
