Your name, address, and date of birth

If you plan to pay cash or use health insurance
-
For insurance pay, please include the following
provider's name,
member ID,
a photocopy of the front and back of the insurance card,
and the name, date of birth, and address of the primary on the insurance.
What's next ...
I will create a profile for you that will allow me to send you the necessary intake documentation to review and complete. This includes my policies and procedures, HIPAA notices, clinical assessments, etc...
As soon as these documents have been completed and, if applicable, the insurance is verified, we can schedule our first appointment\intake.
